We invite all patients from the age of 18 and up to schedule an appointment for evaluation and treatment. At this time, we do not treat ADHD and do not provide prescriptions for narcotics. We follow evidence- based guidelines to provide optimal care for our patients.

You will click on the link in this section to request an appointment. You will then receive our eligibility questionnaire. Please allow 48 hours for our staff to review your questionnaire. If you are an appropriate candidate for our weight loss program, you will receive a confirmation email/ text with our intake forms to be completed prior to your appointment.

During your weight loss consultation, you will be evaluated by our clinician and provided with one month's supply of medication and healthy eating tips to ensure your success. We ask that you plan to visit once monthly for the first 3 months. Visits will be scheduled as indicated thereafter.

We offer the option to receive your medications during your visit which allows you to avoid wait times and shipping fees. This option offers Tirzepatide only and pricing starts at $190 which includes your office visit charge and one month's supply of medication.
